Unified Community Connections provides supported living services in individuals’ private homes throughout Maryland to promote individuality and maximum independence. These supports and services enable people with disabilities to live in homes of their own, independently or with family or housemates of their choosing. They are available up to 24-hours a day, seven days a week.
Supported living services may include but are not limited to coordination of services, training, personal support and supervision. The type and level of support varies widely and is determined by an individual’s needs and preferences.
