Unified Community Connections’ personal supports services enable people living with disabilities to be supported in ways that promote individuality and maximum independence. Personal supports are provided in an individual’s private home and may exceed 82 hours per week within the authorized budget for personal supports with preauthorization from the Developmental Disabilities Administration (DDA).
The type and level of personal supports a person receives varies widely and is determined by each individual’s needs and abilities as outlined in their person-centered plan. Personal supports may include but are not limited to personal care, community integration training, housekeeping, meal preparation and budgeting. Unified’s personal supports program makes it possible for many individuals to live independently or remain living with their family.
