Step 1: Get DDA Approval

Before Unified Community Connections can begin the application process, the individual in need of services must apply and be approved for funding through, the Developmental Disabilities Administration (DDA).

Visit DDA’s website for more information and to complete the DDA application.

Step 2: Complete Unified’s Application Form

Once you have approval, complete the form below to apply for services through Unified.
